just make a php form that takes all that stuff. I wont send it because it uses my work email

making the php submit form takes 5 mins to do either with dropdown or radio buttons. all you need is apache to host it.

I apologize for those who are having trouble with the PDF. I've got a limited amount of time to complete this survey, so I couldn't do anything elaborate. I planned on creating a PHP/MySQL survey, but I don't have enough time.
